The Data Privacy Act of 2012 requires Eternal Gardens to notify you of the fact that it is collecting, using, maintaining and disclosing your personal information and that we may obtain your written consent to do so. • Basic health information inquiry if you have within the last year been confined in a hospital or received any medical or surgical advice or attention.
• Name of your designated person for the Certificate of Ownership.
• ID (which may contain contact information of another person, and your photo identification).

Eternal Gardens and our authorized agents and affiliated service providers use the personal information we collect for the following purposes:

• To provide you with continuous service by up-to-date status of your account.
• To assists you with your payment transactions.
• To respond to your inquiry about current promos and events that may benefit you.
• To meet legal and regulatory requirements and responsibilities.
• To share or disclose information to our affiliated service providers (our affiliated service providers follow same principles in protecting your data as discussed in our DATA SHARING AGREEMENT).
• To process any requests in connection with your account.

This notice and consent procedure has been developed and implemented to protect the confidentiality and security of your personal information. You have the right to request access to your personal information and to request corrections to your personal information to ensure its accuracy and completeness. To make a request for access to personal information we may have collected, disclosed or used about you or to request that your personal information be corrected, please submit your written request to the Data Protection Office – 5th Flr. 2113 Dominga Building III, Chino Roces Ave., cor.
